The CAP administrative staff is more than happy to work with any consultant and/or billing agent a physician has appointed.  In order to release information CAP will need an authorization letter signed by the physician.  The authorization letter should at a minimum include:

• Physician’s full name
• Date authorization will expire
• Scope of what authorization allows
• Individual/Company name of appointed agent

Please keep in mind that even with a signed authorization letter on file, CAP may still require a signature by the contracted provider for some requests to be executed.  Authorization letters without expirations dates will automatically expire after one year from date on letter or date received.
