What should be done if you have an address or tax id change?

Send correspondence to CAP  (fax 512/482-4191) with the below information:

• Physician Name
• Current office and/or billing address including phone and fax number
• New office and/or billing address including phone and fax number
• Current tax identification number
• New tax identification number
• Effective date of new address or tax identification number
• W9
• Sample HCFA with boxes 25, 31-33 completed
